      The origin of microsatellite instability (MIN or RER) has been documented in some familial cancer syndromes and early onset sporadic colorectal carcinoma. MIN can be used for molecular diagnosis in two types of tumors (hereditary nonpolyposis colorectal carcinoma and bladder carcinoma), but for the majority of solid tumors further study is needed to assess the significance of MIN.

      Precancerous hepatic nodules exhibit telomerase activity levels similar to those of hepatocellular carcinomas. However, telomerase activity cannot be used as a marker of malignant transformation in hepatocarcinogenesis because it also may be present in large regenerative nodules.

      The authors report two microscopic pancreatic ductal adenocarcinomas found incidentally at autopsy. The common characteristic of both cancers was the invasion of islets and nerves, which indicated that the cancers were malignant from their inception. These cancers and the altered ductal epithelia adjacent to them showed mutation of the c-Ki-ras oncogene. The results of this study indicate that c-Ki-ras mutation is an early event in pancreatic carcinogenesis and that the most effective way to fight against this cancer is to prevent its induction.

      A freestanding breast clinic's use of fine-needle aspiration cytology and 14-gauge core needle biopsy was found to be a cost-effective way to spare women from unnecessary open surgical biopsies without compromising breast carcinoma detection. Utilization of these methods resulted in a cost savings of > $1,000,000 in 1 year.

      Expression of the neural cell adhesion molecule (NCAM) in human astrocytic tumors was studied and compared with the degree of malignancy of the tumors. To the authors' knowledge, this study provides the first direct evidence that NCAM is down-regulated in the malignant evolution of astrocytic tumors; and it is suggested that reduced NCAM expression might be involved in the development of the biologic malignancy of these tumors.

      In this study of the Groupe d'Etude des Lymphomes de l'Adulte, an extension of the Cox proportional hazards model was used to determine changes in the prognostic value of covariates as time progressed. Performance status, lactate dehydrogenase level, tumor stage, and number of extranodal sites were independently predictive of short term survival; however, only performance status was predictive of a patient's ability to tolerate induction chemotherapy, and only tumor stage was predictive of long term survival.
