Cancer patients who have completed treatment can get help transitioning to the next stage of their survivorship with a new program called Journey Forward. The program's survivorship toolkit includes treatment summaries and detailed follow-up care and health monitoring plans.
Developed jointly by the National Coalition for Cancer Survivorship, the UCLA Cancer Survivorship Center, Well-Point, and Genentech, Journey Forward enables survivors to better advocate for themselves and monitor their health. The survivorship care-plan builder is available to any oncologist, and the medical-history builder helps patients easily record their health histories. Special templates also are available for breast cancer and colon cancer survivors.
