    The stage I and II model was adjusted for National Comprehensive Cancer Network (NCCN) institution, age at diagnosis, histology, resection status, and receipt of radiation; the latter 2 variables were stratified because of nonproportionality. The stage III model was adjusted for NCCN institution, age at diagnosis, receipt of radiation, resection status, receipt of drug therapy, and ECOG performance status; the latter 3 variables were stratified because of nonproportionality. The stage IV model was adjusted for NCCN institution, age at diagnosis, receipt of drug therapy, and Eastern Cooperative Oncology Group performance status; the latter 2 variables were stratified because of nonproportionality.
