      The authors used virtual microscopy in proficiency tests to determine its feasibility for widespread testing. The results indicated the possibilities and limitations of using virtual microscopy as a valid method for proficiency testing to determine cervicovaginal interpretative competence.

      Women with unsatisfactory or low-grade Papanicolaou smears may carry a risk of developing high-grade epithelial cervical lesions at a later date. In a screening program, a subgroup of smears may be diagnosed as unsatisfactory or low grade despite the presence of high-grade findings that are detectable on reexamination.

      The reporting of benign-appearing endometrial cells only in women aged >40 years has led only to a slight increase in the frequency of reporting of endometrial cells in Papanicolaou tests prior to the discovery of endometrial hyperplasia or malignancy with a very large concomitant increase in the frequency of reporting endometrial cells.

      The distinction of primary effusion lymphoma (PEL) from other lymphomatous effusions is difficult on cytologic and a routine immunocytochemical basis alone. Detection of human herpes virus-8, which can be performed by immunocytochemical methods, is an essential confirmatory ancillary test in suspected cases of PEL.

      The reproducibility of the cytologic diagnosis obtained by endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and endoscopic ultrasound-guided fine-needle aspiration biopsy (EUS-FNA) is excellent among pathologists who are experienced in the assessment of these types of samples. Pathologists who are generally experienced but have little experience with EBUS-TBNA and EUS-FNA show a steep learning curve; after a limited teaching session the reproducibility of their diagnoses improves markedly.

      Fine-needle aspiration cytology (FNAC) of head and neck masses in pediatric patients should be included in the clinical diagnostic armamentarium. Pediatric FNAC of head and neck masses confers a high specificity to allow for diagnostic confidence.

      There has been a renewed interest in screening for lung cancer given the detection of highly curable cancers by high-resolution computed tomography (HRCT). The authors determined that diagnosis of these early lung cancers by transthoracic FNA was highly reliable in 53 cases diagnosed by cytology in the baseline screening of 2968 high-risk men and women enrolled in ELCAP at Weill Medical College.

      In a validating cohort study, 100% typing accuracy was found in 23 routinely diagnosed fine-needle aspiration cytologies (FNACs) of the liver differentiating hepatocellular carcinoma from metastatic carcinoma or regenerative nodules using an immunocytochemical panel (HepPar1, α-fetoprotein, BerEP4, CD31, CD68, and Ki-67). An accuracy of 90.3% was achieved in 85 FNACs of the liver using another panel to identify the primary sites of metastatic carcinomas (CK5/6, CK7, CK20, CA 125, thyroid transcription factor-1, and Cdx2).
