1. In the article “Staphylococcus aureus Biofilms: Nemesis of Endoscopic Sinus Surgery,” by Singhal et al., The Laryngoscope, July 2011, Dr. Joshua Jervis-Bardy's name was spelled incorrectly. The corrected name appears below:

2. The authors of the article “Xylitol Nasal Irrigation in the Management of Chronic Rhinosinusitis: A Pilot Study” report an error in the listed concentration of their xylitol nasal irrigation solution. The published manuscript incorrectly listed the use of 12 mg of xylitol dissolved in 240 mL of water to obtain a 5% weight per volume solution. It should have read that the authors used 12 grams of xylitol in 240 mL of water. This was a typographic error. The authors' formulation was verified as correct in association with their hospital pharmacists prior to implementation.
