Unplanned shutdowns plus lack of knowledge equals incidents



Two incidents and a near miss, each precipitated by an unplanned shutdown, are reviewed. In every case, a lack of understanding of the process control and/or chemistry led to an uncontrolled reaction that resulted in process equipment damage and, in one case, injured several people.
