    • Yvonne B. Meserve, C.N.M., M.S. received her diploma from Mercy Central School of Nursing, Grand Rapids, Michigan, A.S. degree from Merced Community College, Merced, California, and her B.S.N. from California State University Fresno, Fresno, California. Her nurse-midwifery and graduate degrees were received from the University of Utah, Salt Lake City, Utah. She has taught maternity nursing at the Intercollegiate Center for Nursing Education, Spokane, Washington. Ms. Meserve has been a La Leche League leader and taught the LaMaze approach to childbirth education. She is currently the Perinatal Supervisor at Big Bear Community Hospital, Big Bear Lake, CA.

The concept of self care education and participation are incorporated into a prenatal record. The client and the nurse-midwife jointly define written needs and goals and record directly on the chart. When opened, the folded record reveals all data necessary to assess pregnancy status and plan care. The outer pages provide demographic data, histories, and physical examinations. Other unique features of the chart include graphs for visualization of fundal height and weight gain patterns and lists for potential and acute problems and needs.
