Although the BJU International has enjoyed a long association with paediatric urology, it is over the last decade that this relationship has particularly flourished as both the speciality and the journal have grown in international stature.

From its inception, the ESPU has been an outward-looking organisation, as shown by the highly successful joint meeting between the ESPU and the Urological section of the American Academy of Pediatrics held jointly in Tours in 2000, and this year's joint meeting with the EAU in Madrid. This enthusiastically international approach is shared by the journal which, in partnership with the ESPU, has welcomed the exciting opportunities created by political change within Europe to contribute to the development of paediatric urology not only in Western Europe but also in Eastern Europe and the countries of the former Soviet Union.

Just as the membership of the ESPU and its role within paediatric urology now extends far beyond Europe, so the BJU International continues to acquire readers and contributors on a truly worldwide scale. For example, in 2002 the journal published original papers from paediatric urologists in no fewer than 22 different countries across five continents.

Superficially this a seems an attractive proposition, but on closer scrutiny it is apparent why the interests of paediatric urology are better served by a world-class journal such as the BJU International, embracing both the adult and paediatric urological disciplines.

First, the practical realities; it is abundantly clear that many North American paediatric urologists would not be prepared to abandon their loyalty to the ‘white’ and ‘gold’ journals. Without a substantial input from North American paediatric urologists it is arguable whether a sufficient volume of high-quality original material could be generated to fill a regular monthly journal devoted entirely to paediatric urology.

However, the BJU International is gratified by the growing number of excellent paediatric papers being received from the USA and Canada, reflecting perhaps the appeal of the journal's wide international readership, coupled with its reader-friendly format and availability of high-quality colour reproduction at no cost to the authors.

Paediatric urologists can be credited with some of the highest quality original research currently being undertaken in urology. These authors merit the recognition and scientific credibility conferred by publication in a major international journal such as the BJU International. Whilst no clinical journal can ever match the impact factors boasted by the major basic science publications, paediatric urologists can derive considerable satisfaction from the fact that the paediatric content of the BJU International is the most extensively cited section of the journal.

In addition to these important practical and scientific considerations there are other compelling arguments for encompassing paediatric and adult urology within the pages of a single journal. Cross-fertilisation of new concepts and technical innovations is far more effective in a journal which facilitates communication between the age-related disciplines. Examples include advances in continent reconstruction (which are widely shared between adult and paediatric reconstructive practice), minimally invasive treatment of stone disease (pioneered in adults but now finding an increasing role in children) and the translation of innovative laparoscopic procedures from adult to paediatric urology.

In addition, the BJU International spans the increasingly important interface of adolescent urology and provides a valuable forum for the publication of long-term outcome studies, of relevance not only to paediatric urologists but also to the adult urologists who care for these patients in later life.

Specialist paediatric urologists sometimes overlook the important contribution made by adult urologists to the general urological surgery of childhood in treating conditions such as the undescended testis. Thus the BJU International particularly welcomes manuscripts relating to the broader aspects of children's urology and which are likely to prove of interest and educational value to this large and important group of general urologists.

The concise, authoritative review articles which are now a lively feature of the journal are intended to enable busy surgeons (of whom the paediatric urologist must surely rank amongst the busiest) to remain abreast of recent developments and current thinking across the urological spectrum. We shall also continue to publish high-quality supplements devoted to paediatric topics, e.g. the excellent forthcoming supplement on Intersex compiled by the international faculty of the ESPU-designated symposium held recently in Lyon.

At a practical level the BJU International’s commitment to paediatric urology also includes publication of the abstract book for the ESPU annual scientific conference, and free advertisements and advance announcements for paediatric urological meetings, courses and workshops, wherever space permits.
