      A 73-year-old man with coronary artery disease, atrial fibrillation, and lung cancer was admitted with recurrent pneumonia and referred for transesophageal echocardiogram for suspected endocarditis by transthoracic echocardiography. The patient had radiation treatment 6 years prior. He subsequently developed esophageal strictures requiring repeated dilatations, and esophageal stenting. Transesophageal echocardiography (TEE) was performed under fluoroscopy for safety. The esophageal stent position was confirmed with fluoroscopy, the TEE probe remained inside distal stent during the procedure. Aortic valve vegetation were found.  This case illustrates TEE may be performed in patients with esophageal stents under careful fluoroscopic guidance.

      Saddle pulmonary embolism is defined as a visible thromboembolus straddling the bifurcation of the main pulmonary artery. It is very unusual to visualize a pulmonary artery thrombus on transthoracic echocardiography. We describe a case of a saddle embolus of the main pulmonary artery visualized by real time three-dimensional echocardiography and successfully treated with intravenous unfractionated heparin, followed by oral anticoagulation achieving a complete dissolution of the thrombus.

      This article presents an adult case of ALVT with aortic ostium lying above the left-noncommissure, which was diagnosed with live three-dimensional echocardiography (3DE). Live 3DE has shown its value in providing more detailed information.

      Unusual Presentation of Cardiac Tamponade after Biventricular Assist Device Implantation: Diastolic Collapse of the Outflow Graft (pages E20–E21)

      We present a 60-year-old patient after implantation of a pulsatile biventricular assist device in whom, on the 10th postoperative day an intrapericardial hematoma was found during echocardiographic examination. A thoracotomy was performed, but 2 days later a pericardial effusion appeared again. Serial daily echocardiographic examinations were performed while the patient remained clinically stable. On the 18th postoperative day, an echocardiographic examination revealed the diastolic collapse of the outflow graft. Repeat thoracotomy was successfully performed. In the case presented, the diastolic collapse of the outflow graft was the most clinically useful echocardiographic fi nding that indicated the presence of significant tamponade physiology requiring surgical intervention with repeat thoracothomy.

    2. Exchange of a Tandem Heart to a HeartMate II Left Ventricular Assist Device Complicated by an Intracardiac Thrombus (pages E22–E23)

      A 63-year-old man suffered an acute myocardial infarction complicated by cardiogenic shock. A TandemHeart left ventricular assist device (LVAD) was percutaneously placed. However, since mechanical support could not be weaned, it was planned to exchange the TandemHeart for a HeartMate II LVAD. Intraoperative transesophageal echocardiography demonstrated a thrombus in the aortic root. As the patients condition acutely deteriorated, the TandemHeart was emergently converted to conventional cardiopulmonary bypass with vacuum-assisted drainage using the existing cannulas. During this transition, the thrombus dislodged and was drawn retrograde through the left ventricle into the LA, where it partially occluded the inflow cannula.

      Parachute mitral valve (PMV) is a rare congenital anomaly of the mitral valve apparatus frequently diagnosed in infants and young children usually as part of Shone's complex (supravalvular mitral ring, subaortic stenosis, and coarctation of aorta). PMV may also rarely occur as an isolated lesion. We herein demonstrated the three-dimensional echocardiographic features of isolated PMV in a 22-year-old man.
