The estimated number of all problematic heroin users was taken from previous work [2], applying additionally a cessation rate of 4% per year between onset and first treatment.

Appendix S1. To facilitate the application of the ‘inclusion function’ for other researchers, a supplementary EXCEL file including all formulas is available online (tabulating numbers and drawing graphs). This file needs, as an input, the date of the selected treatment day, the total number of individuals in substitution treatment and the observed number of individuals who started to use heroin regularly for each year. If the number of overdose deaths during the last decades in the respective region is also entered, the overdose rate is computed and displayed graphically.
