Summary
A 22-year-old woman developed postpartum acute renal failure due to intra-renal intravascular coagulation. Treatment with heparin and streptokinase was unsuccessful. Microangiopathic haemolytic anaemia and persistent heart failure with probable cardiomyopathy were features of her illness, remitting with bilateral nephrectomy and a temporarily successful renal transplant.
The syndrome of postpartum haemolytic uraemia has recently been recognized. Its features, possible pathogenesis and treatment are discussed. The relevant literature is briefly reviewed. Theraphy with heparin has been successful in two reported cases.
