We thank Drs Jayadeva and Albertazzi for their expert comments. We agree with all the points they make, and indeed will be publishing our own findings on these issues in the near future which are entirely in keeping with this.

The application of specialist family planning expertise, however, necessitates that women with sickle cell disease are directed to such services by their haematology and general practitioner physicians, and indeed by gynaecologists who are not confident in this field, and that these colleagues are aware of the positive help which experts in the field can offer to these women. The high rate of unplanned pregnancies demonstrates that this is not currently happening, which is the message we were trying to convey.
