Many cardiac structures (e.g., eustachian valve, Chiari network, pectinate muscles, moderator band, and trabeculations) are identifi ed as normal anatomic variants. The possibility of erroneous interpretation as pathologic states is frequent. We describe a 63-year-old male with three-vessel coronary artery disease who presented with worsening exertional dyspnea. Transthoracic and transesophageal echoardiography revealed moderate mitral regurgitation with elongated and redundant anterior mitral leafl et. A fi lamentous structure attached to the anterior mitral valve leafl et was also seen. During cardiac surgery (bypass grafting and mitral valve repair), the fi lamentous structure was excised and seen as a network attached to both anterior leafl et and its chordae.
