We present a case of a left main coronary artery thrombosis after stent implantation in 42-year-old man. The transoesophageal examination revealed a mobile hyperechogenic structure protruding from the left main coronary artery into left sinus of valsalva. A diffuse akinesis of the left ventricle was noted. Intraoperatively, a thrombus in the left coronary ostium and massive myocardial necrosis was confirmed.
