Some planarian species have the unique ability to regenerate a head, including a functional brain. Here, we summarize the current knowledge on the key steps of planarian head regeneration (head-versus-tail decision, anterior pole formation and head patterning) and their molecular and cellular basis. We discuss the instructive properties of the anterior pole, and a two-step model of regeneration initiation, hypothesizing that wound-induced and existing positional cues interact to detect tissue loss and determine the appropriate regenerative outcomes.
