Relationship among α amylase and carbonic anhydrase VI in saliva, visible biofilm, and early childhood caries: a longitudinal study

This issues Editor's Choice was a very close call between the two caries diagnostic papers, in this issue. They had one remarkable similarity, a recording of the presence of biofilm as a marker of disease risk and activity. All dental professionals know that caries is ‘biofilm-sugar-dependent disease’ mediated by other factors.[1] However, it is my belief that many of our profession concentrate on the sugar aspect of the disease to the detriment of the biofilm aspect. For example improving the removal of biofilm may be a much easier behaviour change to ask people to make than dietary ones. Similarly, when examining patients the information the presence of biofilm at specific sites gives is not utilised as effectively as it can and should be to control and prevent disease.

The chosen paper also looks at the role of saliva, specifically salivary proteins as a risk marker for caries. Again although there are numerous studies on the potential role of the constituents of saliva as a risk marker for caries, we again do not utilise fully this potential.
