Additional Requirements

Author Contributions

Congestive Heart Failure adheres to the authorship guidelines set by the International Committee of Medical Journal Editors (www.icmje.org). If you are invited to submit a revised manuscript for possible publication, you will be asked to submit one form reflecting the contributions of all authors detailing his/her contributions. The forms, which must be uploaded as Supplementary Material Not for Review, will appear online as Supporting information if the article is published. Failure to upload the forms with your revision will block further manuscript processing.

Statement of Financial Disclosure
Should be listed at the end of the manuscript and include specifics on financial or proprietary interest in the subject matter or materials discussed in the manuscript, e.g., employment, consultancies, stock ownership, honoraria. All funding or financial support for the paper received through grants or sponsorship must also be provided here.
If editorial assistance was funded by a third party, the editorial assistance must be disclosed, citing the name and affiliation of the editor and the source of funding for the editorial assistance.

Sample wording:  Editorial assistance was provided by _(insert name)___ of ___(insert company/affiliation name, location)___ ; editorial assistance was funded by __(insert name of sponsor)____.

If no funding exists for any aspect of the article, the statement, “The authors report no specific funding in relation to this research” should be reported on the disclosure section of the article and on the cover page of the article.

Acknowledgments
Should identify anyone who has made substantial intellectual contributions to the manuscript, but who does not meet the criteria for authorship.
Such individuals should be listed at the end of the manuscript and include all relevant information (full names of institutions, companies, and individuals with/without academic degrees). The author is solely responsible for obtaining written permission from those acknowledged and providing them to publisher upon request. Work that is funded (e.g., editorial assistance provided by a corporate sponsor) must be included in the Disclosures section, not the Acknowledgments section.

References
Strict adherence to AMA Style Guide, 10th Edition (Oxford University Press; 2007), Section 3.1—References. Cite references numerically in text (do not use letters); cite unpublished material and personal communications directly in the body of the manuscript and not in the reference list. References should be listed at the end of the manuscript and not in footnote form. The author is solely responsible for the accuracy of references and their use with respect to the supported text.

Pharmaceuticals
Drugs and devices should be referenced by generic name and not by trademark to the extent appropriate.

Acronyms
Acronyms not otherwise listed in the AMA Style Guide should be spelled out within the body of text the first time used.
