Follow‐up biopsies identify high rates of persistent rejection in pediatric kidney transplant recipients after treatment of T cell‐mediated rejection

Follow-up biopsies after treatment for TCMR identify a high rate of persistent rejection (55%) in pediatric kidney transplant recipients. This highlights the importance of follow-up biopsies to identify those with ongoing rejection to guide further management, as well as the need for more effective treatments for TCMR.

New guidelines allow live varicella vaccine (LAVV) for certain children post-transplant based on evidence of LAVV safety. We explored parents' and HCPs' views of new LAVV guidelines in a qualitative study. Findings suggest HCPs should adopt a family-centered approach to communicating changing guidelines that consider parents' approach to adaptive leadership.

A retrospective study found pediatric and young adult abdominal transplant recipients need less than adult-labeled dosing of LCP-tacrolimus to achieve therapeutic tacrolimus levels but require similar conversion ratio from immediate release to LCP-tacrolimus. No safety concerns were noted in either de novo or conversion group.

Data from children transplanted from 2010 to 2019 were analyzed using machine learning. Single ventricle congenital heart disease contributed most to overall risk in the first year, followed closely by mechanical circulatory support at the time of transplant and a history of cardiac surgery prior to being listed for transplant.
